Your First Visit
We ask that you arrive 10 minutes prior to your scheduled appointment to complete our New Patient form.
You can also print and fill out the form in advance to bring to your appointment.
Initial Examination
During your first appointment, your dentist will review your medical and dental history. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening, and any necessary X-rays.
Treatment Plan
After examining your dental history, your dentist will create a comprehensive and personalized dental treatment plan for you.
Insurance & Payment
At Affinity Dental Sherwood Park, we'll help you navigate our options for payment and insurance so you can focus on achieving your dental goals.
Insurance Claims
We can directly bill your insurance company, and our team can help you receive the maximum reimbursement to which you are entitled. Our clinic uses a paperless electronic claims submission service to help you get reimbursed as quickly as possible.
If you require extensive treatment, you have the option of submitting a pre-determination of dental benefits to your carrier to help determine how much reimbursement you may expect. This is not mandatory, but it is an option, and our staff will assist you with this.
Payment Methods
Payment for the full treatment is required at the time of your appointment. We accept payment by cash, debit, or most major credit cards.
Support for Anxious Patients
Some people feel anxious about visiting the dentist and it can prevent them from getting the dental care they need.
At Affinity Dental Sherwood Park, we offer nitrous oxide to help our patients feel more relaxed during long or uncomfortable procedures.

How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us. Please let us know about any change of plans as soon as you can, so we can offer the space in our schedule to another patient.

Does your clinic have on-site parking?
Yes, we have free on-site parking available for all our patients. We are also easily accessible by public transit.

What are our infection prevention and control measures?
At Affinity Dental Sherwood Park, your health and safety are of the utmost importance. We pride ourselves on staying current with the guidelines that govern infection prevention and control in Alberta, and we implement changes in our practice to align with changes to the guidelines as they occur.
Some of the measures we take to protect the health and safety of patients and staff include:
- Staff vaccination against hepatitis B virus (HBV)
- Hospital grade disinfectant used to clean the room after each patient
- Medical grade instrument cleaner and ultrasonic instrument cleaning equipment
- Daily, weekly, and monthly maintenance of autoclaves and all other equipment completed and documented
- Sterilisers tested daily with biological monitors
- Sterilisers tested with chemical monitors in each load
- Every individual instrument pack has a chemical indicator within the pack
- Headpieces are flushed and dried after each use
- Sterile gloves and drapes are used for implant and grafting procedures.
